Metastatic Lobular Carcinoma of the Breast in Pleural Fluid: Diagnostic Pitfall for Mesothelial Cells

چکیده
Cytomorphologic patterns of breast carcinoma in effusion include three-dimensional groups of tightly packed clusters for invasive ductal type and single files with cytoplasmic signet ring-like cells for invasive lobular type. A mesothelial-like cell pattern has been described for metastatic lobular carcinoma in pleural fluid. We report a case of a patient with a remote history of lobular carcinoma of the breast who presented with hypocellular pleural effusion with the mesothelial-like cell pattern. Without the previous clinical history and the help of marker studies, the mesothelial-like cell pattern of metastatic lobular carcinoma to pleural fluid may be difficult to distinguish from benign reactive/atypical

Interpretation of pleural biopsy specimens and aspirates with the immunoperoxidase technique.
In pleural biopsy specimens and histological sections from the fibrin clots of pleural fluid aspirates it may be difficult to distinguish reactive mesothelial cells from malignant mesothelial cells and metastatic carcinoma. Reactive pleurisy with effusion is usually associated with loss of cohesion and exfoliation of mesothelial cells, which is consistent with the hypothesis that they act as fa...
متن کاملNasopharyngeal Carcinoma with Cystic Cervical Metastasis Masquerading as Branchial Cleft Cyst: A Potential Pitfall in Diagnosis and Management
Introduction:Most metastatic lymph nodes from head and neck malignancy are solid. Cystic nodes are found in 33% - 61% of carcinomas arise from Waldeyer’s ring, of which only 1.8% - 8% originate are from the nasopharynx. Some cystic cervical metastases were initially presumed to be branchial cleft cyst. This case report aims to highlight the unusual presentation of cystic cervical metastasis sec...

Dear Dr. Bedrossian: I would like to offer an explanation as to how the nodules of so-called nodular histiocytic hyperplasia (NHH) form in some specimens of pleural, peritoneal, and pericardial fluid. Such nodules were well-illustrated by Drs. Choi and Song in their recently published article in Diagnostic Cytopathology.1 They demonstrated that these nodules are composed of compact aggregates o...
متن کاملHistomorphological and Immunohistochemical Analysis of Pleural Neoplasms
Background & objective:Primary pleural neoplasms are rare entities compared with the pleural involvement by metastatic carcinoma. The current study aimed at investigating the complete spectrum of pleural neoplasms and differentiating between them with the aid of immunohistochemistry (IHC). Methods: Consecutive pleu...

Serous effusions are frequently a clinical manifestation of metastatic disease, with lung, breast and ovarian carcinoma and mesothelioma leading the list. The diagnosis of malignant effusion signifies disease progression and is associated with a worsening patient prognosis. The ability to grow in a dense exudative fluid suggests that the malignant cells are capable of acquiring nutrients, survi...
